Rustin Hockey Registration 2011-2012 School Year

REGISTRATION:
Tuesday, May 17th- 5:00-7:00pm in the Ice Line Party Rooms.
Registration for all members in Middle School, High School and Girls.
$250 initial payment due with registration paperwork

MIDDLE SCHOOL EVALUATIONS:
Tuesday, May 17th and Wednesday, May 18th from 6:00-7:15 pm.
(Current grades 4-7 are required to attend both sessions in full equipment)

HIGH SCHOOL EVALUATIONS:
Will take place during September training camp. All High School players are required to register on May 17

Initial Deposit: $250 for Middle School and High School
$200 Elementary Deposit
Deposit is due with registration paperwork.

Copy of Birth Certificate (if new Rustin player)
